How to Submit an Assumption Life Claim
When you're ready to start the process of filing a claim, the forms can look quite daunting and complicated.
If you follow the steps listed below, filing your claim will be a breeze.
1. Determine the Correct Paperwork and Documents
The type of paperwork you fill out will be dependent on your individual situation.
One of your first steps is to determine what paperwork you need. Once you have identified what forms you need you can access them here.
The Document Centre is where you will be able to find any forms needed. You will also need to provide different documents based on the claim you are filing.
Keep in mind that the paperwork listed for each type of claim is only the basic paperwork. You may need to provide additional documentation per request before a decision is made.
Life Insurance Claim
When filing a life insurance claim the beneficiary or claimant needs to complete the Beneficiary's Statement. This claim also gets referred to as the Claimant's Statement.
Documents required for this include:
- Physician's statement (this isn't required for Dependent Life Claims)
- Policyholder's statement
- Funeral director's statement or provincial death certificate
- Birth certificate of the deceased
Optional Life Insurance Claim
With an Optional Life Insurance Claim, the same document will get filled out. You will also need the following documents:
- Physician's statement (this isn't required for Dependent Life Claims)
- Policyholder's statement
- Funeral director's statement or provincial death certificate
- Birth certificate of the deceased
If you have increased your insurance or enrolled in insurance less than two years ago you will need an autopsy report and/or the medical examiner's report.
Accidental Death Claim
If you're filing an accidental death claim you will need to fill out the accidental dismemberment forms. The documents you need to include are as follows:
- Physician's statement (this isn't required for Dependent Life Claims)
- Policyholder's statement
- Funeral director's statement or provincial death certificate
- Birth certificate of the deceased
- Police report
- Coroner's report and/or the autopsy report
2. Get the Correct Signatures
Getting the correct signatures requires knowing who the beneficiaries are. Beneficiaries will need to fill out the paperwork and sign.
If there is more than one beneficiary you will need a signature from all beneficiaries and their addresses. If the beneficiary is a minor then the forms will need to get completed by their guardian or another person authorized by law.
If this is the case a certified copy of the Letters of Guardianship needs to get submitted as well.
If any named beneficiaries are deceased then proof of death will be required.
If the deceased did not have a beneficiary the forms can be completed a couple of different ways.
If the beneficiary is the estate of the life insured then the statement will need to be filled out by the executor of the will.
A certified copy of the notarial will is also required for this. If the person did not have a will then the statement gets filled out by the administrator of the estate.
In this circumstance, you will need a notarial copy of the Letter of Administration. If you are in Quebec then this statement needs to get completed by the heirs of the deceased.
In addition, a declaration regarding heirs will need to be submitted.
3. Provide Beneficiary's Social Insurance Number (SIN)
When the beneficiary or claimant files a claim they will need to provide their SIN. This is requested when more than 50 dollars in interest on the death benefit amount gets paid.
If the estate of the deceased is the claimant then the SIN of the deceased individual is required.
4. Pay Fees
If there are any fees involved it is the responsibility of the beneficiary to pay them. This includes fees for documentation filled out by the physician of the deceased.
5. Submit Paperwork
